Key Responsibilities:
- Assist patients with personal care tasks, including bathing, grooming, and oral hygiene.
- Ensure adherence to safety protocols and care plans while providing assistance.
- Deliver care that aligns with the patient's needs and promptly notify the RN of any observed changes in patient condition, documenting all relevant information accurately.
- Complete documentation in accordance with organizational standards.
- Support patients with mobility and transfers.
- Conduct range of motion exercises.
- Maintain a clean and safe environment for patients, including changing bed linens.
- Assist families with light housekeeping tasks.
- Support patients in self-administering medications and remind them of medication schedules.
- Prepare simple meals according to dietary guidelines and assist patients during mealtime as needed.
- Provide care for foley catheters and colostomy bags, ensuring proper hygiene.
- Perform peri care and measure urine output.
- Assist with ostomy care, ensuring cleanliness around the stoma.
- Complete other duties and projects as assigned.
Demonstrate flexibility in job responsibilities across all areas. Adhere to HMH's organizational competencies and standards of behavior. Physical requirements include lifting a minimum of 50 lbs., pushing and pulling a minimum of 50 lbs., and standing for a minimum of 8 hours daily.
