Key Responsibilities:
The Home Study and Post-Release Services (HSPRS) Case Manager Level 2 is responsible for delivering comprehensive case management and support services to unaccompanied immigrant minors transitioning from federal custody. This role emphasizes a client-centered and trauma-informed approach in collaboration with program staff and supervisors.
Primary Duties Include:
- Conducting home visits and telephonic outreach to evaluate the safety and suitability of child placements with caregivers while facilitating connections to community resources.
- Providing ongoing case management, focusing on screening and referrals to meet the child's diverse needs, including education, health, legal matters, and personal development.
- Performing thorough assessments in a child-focused, trauma-informed manner, ensuring accurate documentation in an online case management system.
- Delivering crisis intervention services as necessary or linking families to appropriate support services during emergencies.
- Building and maintaining relationships with key governmental and community organizations to enhance service delivery.
- Adhering to program policies and procedures to ensure operational efficiency.
- Meeting or exceeding productivity expectations consistently.
- Other responsibilities as assigned.
Qualifications:
Applicants should possess a minimum of one year of relevant experience working with vulnerable or migrant populations, with three years preferred. Proficiency in both Spanish and English, along with excellent communication skills, is essential. Candidates must demonstrate the ability to manage confidential information responsibly and be willing to travel domestically up to 60% of the time. Proficiency in Microsoft Office applications is required.
Education: A Bachelor's degree in education, psychology, sociology, or a related behavioral science field is necessary.
Attendance: Regular and acceptable attendance is required as determined by the employer.
Licenses: A valid driver's license with a clean record is mandatory.
Vehicle: Daily access to a vehicle is required.
Travel: Availability and willingness to travel to various locations as needed for business purposes.
