Key Responsibilities
The Registered Nurse Case Manager will take full nursing responsibility for implementing the Plan of Care for each hospice patient. Your duties will include:

  • Conducting comprehensive nursing assessments

  • Collaborating effectively with patients, families, and the healthcare team

Experience and Licensing Requirements
To qualify, you should have at least one year of clinical experience. Additionally, you must possess:

  • Current RN licensure in your state of practice
  • Current CPR Certification
  • A valid driver's license and vehicle insurance
  • Access to reliable transportation
