What You'll Do:
  • Deliver Empathetic Care: Provide compassionate and personalized care to patients in their homes or facilities, addressing their physical, emotional, and spiritual needs.
  • Assist with Daily Living: Help patients with activities of daily living, pain management, and symptom control, ensuring their comfort and well-being.
  • Support Patients and Families: Offer emotional support and companionship to patients and their loved ones, fostering a sense of connection and community.
  • Coordinate Care: Collaborate with the interdisciplinary hospice team to ensure seamless care coordination and effective communication.
  • Document Patient Records: Maintain accurate and detailed patient records, documenting progress, and any changes in their condition.
About You:
  • Certified Nursing Assistant (CNA) Certification: Possess a valid CNA certification and a deep understanding of end-of-life care principles.
  • Excellent Communication Skills: Demonstrate exceptional communication and listening skills, able to effectively interact with patients, families, and healthcare professionals.
  • Compassion and Empathy: Show a genuine commitment to providing empathetic care, understanding the unique needs and challenges of each patient.
  • Team Player: Be a collaborative team member, working effectively with the hospice care team to achieve exceptional patient outcomes.
