Key Responsibilities
A Support Specialist in Care Management plays a vital role at AHF by leveraging extensive experience in care coordination and social services. This position requires a deep understanding of the social factors affecting the HIV community, as well as the obstacles faced in adhering to care plans. By engaging with patients, the Support Specialist applies a compassionate, knowledgeable, and supportive approach. Coupled with a solid grasp of the Ryan White Program, the Support Specialist collaborates with the Care Management team, Healthcare Center personnel, and medical providers to ensure that every Ryan White patient receives the necessary support and access to resources.
This role encompasses providing education, conducting screenings, and facilitating assistance to ensure eligibility for the program is comprehended and attained. Working directly within the Healthcare Center, the Support Specialist reviews patient records and progress notes to ensure meaningful interactions and maximum support. Establishing regular communication with patients based on their unique needs enables the Support Specialist to understand their circumstances and work towards eliminating barriers to achieving their Service/Action/Care Plan objectives.
By utilizing your care management expertise, you will enhance AHF's reputation, meeting patients where they are, identifying strengths, and fostering growth to help them reach their aspirations. Your collaborative and engagement skills will embody the essence of the Ryan White Care Act, serving as a safety net for uninsured HIV/AIDS patients within our most vulnerable populations. Your interactions with patients are crucial to AHF's success in fulfilling the requirements of our Ryan White contract, as well as contributing to the measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

Licenses and Certifications: A Bachelor's degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or Human Services with a minimum of 1 year of Case Management experience is required.
Familiarity with Ryan White Services is preferred.
