Job Summary

We are seeking a highly organized and detail-oriented Patient Navigator to join our team at Healogics, LLC. As a Patient Navigator, you will play a critical role in ensuring the smooth operation of our outpatient wound care centers.

Key Responsibilities
  • Coordinate with center leadership to maximize daily patient census and optimize resource allocation.
  • Participate in staff meetings to support key functions within the center and contribute to a collaborative team environment.
  • Provide exceptional patient care by greeting patients and visitors, answering calls, and routing them to the appropriate staff member.
  • Maintain accurate and up-to-date patient communication records, including scheduling, rescheduling, and appointment reminders.
  • Verify patient insurance information, obtain pre-certifications and pre-authorizations, and enter data into relevant databases.
  • Collect and enter patient charges, reconcile charges as directed, and prepare monthly patient survey data for transmission to providers.
  • Assemble new patient charts, maintain and file existing patient charts, and conduct regular spot checks to ensure data completeness and signatures.
  • Coordinate and schedule ancillary testing with other hospital departments and arrange for patient transportation as needed.
  • Maintain office equipment and supplies, as well as medical supplies, and perform other duties as required.
Requirements
  • High School Diploma or General Education Development (GED); Associate's degree preferred.
  • Minimum of two years of office administration experience, preferably in a medical setting; prior medical coding experience preferred.
Preferred Qualifications
  • Proficient in Microsoft Office (Word, Excel, Outlook).
  • Excellent customer service, interpersonal, and communication skills.
  • Strong organization and time-management skills.
  • Ability to type 60 words per minute (wpm).
  • Basic math skills.
  • Attention to detail and ability to maintain confidentiality.
  • Ability to work in a fast-paced environment and manage multiple projects simultaneously.
Work Environment

This position requires working in a patient care environment with frequent interaction with patients, staff, and other healthcare professionals.

The hourly rate for this position generally ranges between $23.39 per hour, based on qualifications, experience, and operational needs.
