The ideal candidate will have a Bachelor's degree in Social Work or Psychology and at least one year of experience working with individuals aged four and above in a mental health setting. A strong understanding of community resources and procedures, as well as evidence-based psychotherapeutic techniques, is essential for success in this role.
Responsibilities:
- Assess patients' current psychosocial and medical needs and make appropriate referrals.
- Collaborate with the interdisciplinary treatment team to develop and implement effective treatment plans.
- Conduct daily living groups, psychoeducational groups, and patient/family education sessions.
- Provide discharge planning services, including aftercare arrangements and communication with outpatient providers.
- Maintain accurate and concise documentation in patient records.
Qualifications:
- Bachelor's degree in Social Work or Psychology from an accredited college/university.
- Licensure eligible preferred.
- Minimum one year experience working with individuals aged four and above in a mental health setting or an equivalent level of education and experience.
Estimated Salary:$50,000 - $65,000 per year, depending on experience.
