Outpatient Registered Nurse Care Manager

We are seeking a highly skilled and compassionate Outpatient Registered Nurse Care Manager to join our team at St. Luke's University Health Network. As a key member of our care team, you will play a vital role in coordinating care for patients on providers' daily schedules and proactively managing and coordinating care for patients not on the schedule.
Key Responsibilities- Coordinate care for patients on providers' daily schedules, ensuring seamless transitions and effective communication among care team members.
- Proactively manage and coordinate care for patients not on the schedule, developing individualized treatment plans and ensuring timely follow-up appointments.
- Work collaboratively with patients, providers, and care team members to develop and implement comprehensive care plans, including follow-up appointments, labs, and other necessary interventions.
- Communicate effectively with patients and their families, ensuring they understand discharge instructions, medication regimens, and other essential information.
- Facilitate information flow between hospitals, long-term care facilities, home health representatives, and the patient's primary care team.
- Identify high-risk, high-need patients and work with providers and care team members to implement best practices in preventive services, chronic care, and disease management.
- Utilize electronic health records, chronic disease registries, and other quality reporting software to manage patient care and populations.
- Ensure patient adherence to medical plans of care, including preventive and disease-specific screenings, interventions, and treatment goals.
- RN degree and license for the appropriate state (PA & NJ) required.
- 3+ years of direct patient care experience.
- Current CPR or equivalent certification.
- Proficient in Excel and Word.
