Job Summary

The Physical Therapist is a key member of the RiverStone Health team, responsible for providing high-quality physical therapy services to patients in their homes. As a Physical Therapist, you will assess, plan, and implement individualized rehabilitation programs to improve mobility, relieve pain, and increase strength in patients with various medical conditions.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to achieve optimal outcomes.
  • Administer manual exercises, massage, and other physical therapy interventions to improve patients' strength, mobility, and function.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
  • Monitor patients' progress, adjust treatment plans as needed, and document outcomes in a timely and accurate manner.
  • Provide education and training to patients and their caregivers on proper techniques, exercises, and self-care strategies to promote independence and prevent complications.

Requirements:

  • Graduate of an accredited Physical Therapy program.
  • Current licensure as a Physical Therapist in the State of Montana.
  • Experience in home health care or similar settings preferred.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
