Position Overview

The Senior Care Coordination Manager serves as the primary leader of the service team. This role is essential in delivering direct services and overseeing the daily operations of the care team. The Senior Care Coordination Manager is tasked with orchestrating the care of clients across various service providers, both within the organization and externally. Under the guidance of the Program Coordinator, this position is responsible for developing comprehensive care plans that align with health plan requirements and address identified challenges. The Senior Care Coordination Manager will act as the main liaison for all client service providers, including but not limited to primary care physicians, housing service providers, substance use disorder service providers, and natural support systems.

Key Responsibilities

The primary duties include, but are not limited to:
  • Act as the Senior Care Coordination Manager
  • Ensure proper maintenance of client records and timely completion of documentation
  • Champion the client's voice and choices
  • Facilitate coordination among additional service providers to meet all client needs
  • Support clients in fulfilling basic needs, including finding food resources and applying for benefits
  • Guide clients in developing self-sufficiency skills
  • Mentor clients in acquiring and enhancing daily living skills necessary for independent living
  • Supervise a team of 5 multidisciplinary professionals
  • Gather and report data and outcomes to the Program Manager
Required Qualifications (Knowledge, Skills, and Abilities)
  • Bachelor's Degree in social services or a related field is mandatory
  • Capability to bill Medi-Cal
  • A minimum of 3 years of experience in housing-related social services
  • Clean driving record
