Description

As a skilled Physical Therapist, you will provide intermittent services to patients in their homes, in accordance with physician orders. Your primary responsibilities will include:

  • Providing initial and ongoing comprehensive assessments of patients' needs, including OASIS assessments.
  • Assisting in the development and implementation of the interdisciplinary patient care plan, as it pertains to Physical Therapy.
  • Assisting physicians in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
  • Consulting and familiarizing staff regarding the established plan.
  • Teaching, supervising, and counseling patients and their families in the physical therapy program.
  • Instructing patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
  • Providing the equipment required to carry out the plan.
  • Arranging for out-patient services for procedures that cannot be performed in the home.
  • Recording and reporting patients' reactions to treatment and any changes in their condition to physicians.
  • Communicating with physicians at least every 30 days or whenever changes occur, regarding patients' status and obtaining additional orders if necessary.
  • Participating in case conferences, physical therapy staff meetings, Agency activities, and in-service programs as necessary.
  • Supervising Home Health Aides and Physical Therapy Assistants as per Agency policy.

Qualifications

To be successful in this role, you must have:

  • A Physical Therapist education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or its successor organizations.
  • Passed an examination for Physical Therapist approved by the state of Florida.
  • Licensure or registration by the state of Florida.
  • At least one year of institutional/home health experience.
  • A criminal background check.
  • Current CPR certification.

Job Conditions

The ability to drive and access patients' homes, which may not be routinely wheelchair accessible, is required. You must also have sufficient hearing, eyesight, and physical dexterity to perform a physical assessment of patients' conditions and perform patient care. On occasion, you may be required to bend, stoop, reach, and move patient weight up to 250 pounds, lift and/or carry up to 30 pounds. Effective communication in English is also essential.
