Position: Registered Nurse Case Manager
Employment Type: Full Time, Days
Position Overview:
- Delivering comprehensive care to designated patients.
- Conducting thorough evaluations for assigned patients in accordance with care model standards.
- Performing acute assessments to determine patient health status.
- Emphasizing early detection of health changes alongside effective management of chronic illnesses.
- Completing assessments for members within 72 hours post-discharge from emergency departments, hospitals, skilled nursing facilities, or home health episodes to ensure ongoing stability and care continuity.
- Facilitating effective communication during care transitions.
- Promoting advanced care planning and end-of-life discussions for patients in need, which may involve initiating and providing palliative care services as required.
- Documenting patient evaluations and care plans in the electronic medical record system.
- Reviewing laboratory results and other diagnostic tests for necessary interventions.
- Collaborating with utilization management, primary care providers, specialists, and other interdisciplinary team members.
- Informing family members of any significant changes in patient condition.
- Exhibiting strong communication skills with a focus on customer service and patient needs.
- Assisting with patient emergencies as necessary.
- Recommending and/or conducting specific educational sessions for patients and caregivers as appropriate.
- Performing additional responsibilities as assigned.
Qualifications:
- High school diploma or equivalent required.
- Current, unrestricted Registered Nurse license in the state of practice or ability to obtain and provide proof of license renewal biennially.
- Certification as a Registered Nurse from a recognized national credentialing organization.
- Preferred clinical experience in adult, family, or geriatric care settings.
- Experience in home health or hospice care is preferred.
- Willingness to be available for on-call duties as assigned.
- Dependable, adaptable, and capable of collaborating effectively with nursing staff and other departments while demonstrating understanding, patience, and tact in interactions with patients, families, and healthcare professionals.
- Ability to make informed decisions regarding nursing care, recognizing the potential serious implications of errors.
- Capability to work effectively under pressure, solve problems, and perform various tasks.
- Must maintain a professional appearance and adhere to dress code standards, including wearing a name tag identifying oneself as a Registered Nurse, while ensuring personal hygiene is upheld.
