We are seeking a skilled Registered Nurse to join our team at AT Home Care & Hospice. As a PRN Registered Nurse, you will play a vital role in providing high-quality care to our patients and their families.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine their care needs
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
- Document patient progress and outcomes in a timely and accurate manner
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Graduate of an accredited school of nursing
- Current Registered Nurse licensure in the state
- American Heart Association CPR certification
- One year of hospice experience preferred
- Excellent observation, verbal, and written communication skills
- Problem-solving skills and basic math skills
