Key Responsibilities:
  • Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders.
  • Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, and modalities.
  • Evaluate patient progress and adjust treatments accordingly.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
  • Effectively communicate with supervisors and other healthcare team members regarding patient progress, barriers, and treatment plans.
  • Provide comprehensive discharge summaries of services provided.
Requirements:
  • Licensed Physical Therapist
  • Experience working with patients with cognitive and/or ADL physical functioning disorders
  • Ability to work effectively in a team environment
  • Strong communication and interpersonal skills
