Home Health LPN Position

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at NurseCore in the Naples area. As a home health LPN, you will have the opportunity to provide one-on-one care to patients in their homes, making a meaningful difference in their lives.

Responsibilities:

  • Provide high-quality patient care in a home setting
  • Develop and implement comprehensive care plans in collaboration with patients and their families
  • Conduct patient and family education on disease processes and care needs
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Mentor and educate colleagues to enhance patient outcomes
  • Document and report patient progress and changes in condition

Qualifications:

  • Current LPN licensure in the State of Florida
  • 1-year experience as an LPN in a supervised setting or completion of our Nurse Residency Program
  • CPR certification in compliance with American Heart Association standards
  • Negative TB/PPD or Chest X-Ray within the last 12 months
  • Successful completion of skills assessment specific to areas of experience
