Job Title: LPN Homecare Professional

We are seeking a skilled LPN Homecare Professional to join our team at Maxim Healthcare Services. As a LPN Homecare Professional, you will be responsible for providing high-quality patient care in a home setting.

Job Summary:

The LPN Homecare Professional will work with patients in Grand Ridge, IL, providing skilled nursing care and services as directed by contract and/or program. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently.

Responsibilities:
  • Provide skilled nursing care and services to patients in a home setting
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Monitor patient progress and adjust care plans as needed
  • Document patient information and care provided in a timely and accurate manner
Requirements:
  • Graduate of an accredited practical nurse or vocational nursing program
  • Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the state of Illinois
  • One year prior professional nursing experience preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
