As a key member of the pharmacy team, the Clinical Pharmacist Specialist in Hematology Oncology will provide comprehensive pharmaceutical care within the hematology and oncology departments. This role encompasses leadership in clinical pharmacy activities, ensuring the delivery of high-quality patient care and promoting safe medication practices.
Key Responsibilities
- Clinical Leadership: Lead pharmacy clinical initiatives and collaborate with healthcare professionals to achieve departmental goals.
- Patient Care: Deliver specialized pharmaceutical care to patients undergoing treatment for hematologic malignancies and solid tumors, ensuring optimal medication therapy management.
- Interdisciplinary Collaboration: Work closely with the medical team to review patient treatment plans, providing drug information and recommendations based on evidence-based practices.
- Education and Training: Participate in the training and development of pharmacy staff, residents, and students, enhancing their clinical knowledge and skills.
- Quality Improvement: Engage in quality assurance activities, including medication utilization reviews and adverse drug event reporting, to improve patient safety and therapeutic outcomes.
- Formulary Management: Assist in the evaluation and management of the pharmacy formulary, ensuring the safe and effective use of medications.
Qualifications
- Pharm.D. or equivalent degree in pharmacy.
- Preferred completion of a pharmacy residency program.
- Minimum of three years of relevant clinical pharmacy experience.
- Licensure as a Clinical Pharmacist in the state of Michigan or eligibility to obtain.
