Nexus HR is seeking a skilled Registered Nurse to serve as a Clinical Manager for a reputable, locally owned and operated hospice company in the metropolitan Atlanta area.

About the Role:

The ideal candidate will provide Hospice care to terminally ill patients 24 hours a day, 7 days a week, with on-call responsibilities at least once a month. The Clinical Manager will oversee and supervise all Hospice care to patients and families in all settings, ensuring safe and therapeutically effective service.

Key Responsibilities:

  • Supervise the planning, coordination, and delivery of interdisciplinary group interventions.
  • Assign patients/families to appropriate staff members.
  • Participate in and provide leadership for regular interdisciplinary group meetings, development, review, and revision of patient/family plans of care, facilitation of problem-solving sessions, and delivery of team support.
  • Implement and monitor compliance with policies and procedures.
  • Serve as a member of the management team to address problems associated with the delivery of services and staff development needs, suggest additions/changes in policies and procedures, and provide peer input to enhance and improve the management function.
  • Supervise the maintenance of medical records, ensuring accuracy, completeness, and compliance with licensing regulations, certification standards, legal, and ethical standards.
  • Provide supervision of support and clerical staff in cooperation with other members of the management team.
  • Oversee clinical supervision of nursing personnel, Hospice Aides, and Homemakers.
  • Conduct supervisory visits for clinical observation of performance of assigned team members.
  • Provide information for inclusion in written performance evaluations for all staff assigned to the team in cooperation with appropriate members of the management team.
  • Provide leadership and counsel to nursing personnel to measurably enhance staff development.
  • Identify the need for staff education and assist in the development of inservice programs for interdisciplinary groups.
  • Assist in education/training programs.
  • Participate in developing standards that ensure safe and therapeutically effective service to patients and families.
  • Participate in developing objectives for Hospice.
  • Ensure objectives are implemented.
  • Consult with the Administrator to determine a staffing pattern that will accomplish stated objectives and promote maximum utilization of personnel.
  • Responsible for recruiting, hiring, evaluating, and terminating service personnel.
  • Participate in planning for the orientation of new employees and conduct selected orientation classes.
  • Plan and arrange for consultation needs of staff; prepare and maintain current policies and procedures that meet Medicare, Medicaid, accreditation standards, state, and federal laws, etc., and implement; revise concurrently.
  • Display a willingness to support policies and procedures and use appropriate channels for change of such policies; establish criteria and procedures for selection, promotion, and termination of employment of service personnel.
  • Participate in establishing the functions and qualifications for each service position; coordinate interdisciplinary team services.
  • Assure ongoing assessment of patient/family needs and implementation of interdisciplinary group plan of care.
  • Assure physician approval of plans for continuity of medical care.
  • Provide individual or group support concerning job-related stress or issues.
  • Organize Hospice to delineate and delegate authority, functional responsibility, lines of relationship, and communication to provide safe and therapeutically effective service.
  • Participate in coordinating Hospice's services with services of other community agencies.
  • Serve as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation, and job performance.
  • Observe confidentiality and safeguard all patient-related information.
  • Accept responsibility for regular attendance and punctuality, fulfill job requirements without regard to time involved.
  • Serve as a resource person to employees.
  • Develop a cooperative relationship and communicate effectively and professionally with physicians.
  • Investigate and report any problem relating to patient care or conditions that might harm the patient and/or employee well-being.
  • Immediately report any accident, incident, lost articles, or unusual occurrence to the Administrator.
  • Attend pertinent continuing education programs other than routine inservices and share information with staff.
  • Work with the Administrator in identifying budgetary requirements and determining appropriate use of allocations.
  • Monitor equipment for appropriate use and take steps to keep misuse to a minimum.
  • Oversee Hospice's ongoing Quality Assessment/Performance Improvement (QAPI) program.
  • Provide 24-hour/day, 7-day/week on-call coverage.
  • Plan and supervise the Hospice.
  • Ensure that patients' plans of care are developed, implemented, and evaluated.
  • Conduct patient case conferences, in-services, staff meetings, and maintain documentation; participate in community education projects.
  • Ensure that all necessary supplies and equipment are available.
  • Be normally available at all times during and after operating hours; may designate a qualified temporary replacement if unavailable.
  • Ensure compliance with federal, state, and local rules, accreditation standards, and Hospice policies in all patient care aspects of the Hospice.
  • Oversee staffing and assignments.
  • Address and correct all patient concerns.


Requirements:

  • Currently licensed as a Registered Nurse in Georgia.
  • Two years of hospice or home health experience is preferred.
  • Minimum 3 years of experience as a Registered Nurse.
  • Current valid driver's license.
  • Must provide evidence of annual TB test and other state-required tests or examinations.
  • Reliable transportation with appropriate insurance coverage.
  • Great communication skills (verbal and written).
  • Able to work in two Atlanta, GA, or in Alpharetta, GA.
