Personal Care Specialist for High Acuity Teenager

Aveanna Healthcare is a leading provider of home healthcare services, and we are seeking a compassionate Personal Care Specialist to join our team. This position offers a unique opportunity to make a meaningful difference in the lives of medically fragile patients.
Job SummaryWe are looking for an experienced LPN/LVN to provide high-quality care to a teenager with tracheostomy and ventilator dependency. The ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for delivering exceptional patient care.
Responsibilities- Implement physician-prescribed plans of care to ensure optimal patient outcomes
- Conduct patient assessments and coordinate care with interdisciplinary teams
- Administer prescribed medications, treatments, and therapies as directed by physicians
- Maintain accurate and timely documentation of patient care in electronic records
- Provide health education and training to family members on disease management and self-care techniques
- Accept and write verbal orders from physicians, obtaining co-signatures from case managers as required
- Follow up with physicians, clients, family members, staff, and case managers on patient concerns and changes in condition
- Active, unencumbered RN/LPN/LVN license in the state where you will practice
- Current CPR certification from the American Red Cross or American Heart Association
- TB skin test within the last 12 months
- Six months of prior hands-on nursing experience preferred, but not required
- Reliable transportation for travel to client locations
