- Conduct patient and family education on disease management, medication adherence, and end-of-life care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Develop and maintain effective relationships with patients, families, and healthcare teams to ensure a positive patient experience.
- Current RN licensure in the state of Washington.
- Minimum of three years of experience in hospice, home health, medical-surgical, or skilled nursing facility settings.
- BSN or advanced degree preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Computer proficiency and experience with electronic medical records.
