Job Responsibilities
  • Complete admissions, social services evaluations, provide referrals, and maintain communication with residents and families
  • Assist guests with financial and legal matters, such as applying for pensions or referring them to lawyers or funeral homes for pre-planning arrangements
  • Provide discharge planning services, including placing guests on waiting lists for community congregate living, arranging home care services, and assisting with transfer arrangements to other facilities
  • Provide counseling services and psychiatric medication management as indicated
  • Ensure accurate and timely documentation in MDS care plans
