The Sigma Healthcare team is seeking a qualified Physical Therapist to provide services in a contracted healthcare facility.

Work Schedule: Days, Monday to Friday

Position Overview:
  • The Physical Therapist is responsible for initiating and implementing physical therapy programs, while also educating other staff and family members about the therapy process and goals for patient care.
Key Responsibilities:
  • Deliver physical therapy services in accordance with a physician's established care plan.
  • Conduct assessments to evaluate the patient's therapeutic, rehabilitative, and functional status, contributing to the overall care plan.
  • Evaluate the home environment and assess muscle strength, mobility, gait, and range of motion, focusing on comfort for hospice patients.
  • Direct and oversee physical therapy treatments.
  • Instruct patients and caregivers on the proper use and maintenance of therapeutic devices.
  • Identify priority needs for physical therapy interventions.
  • Communicate with physicians regarding patient responses to treatment and any changes in condition.
  • Provide training for patients on the use of prosthetic devices when necessary.
  • Recognize the needs of patients and families for additional home health services and make referrals as appropriate.
  • Prepare and submit clinical and progress reports based on goal attainment.
  • Engage in discharge planning for patients.
  • Facilitate physical therapy consultations for families and caregivers as needed.
  • Conduct in-service education sessions for nursing staff as required.
  • Participate in peer review and consultation processes.
Qualifications:
  • Current BLS/BCLS certifications.
  • Valid Washington state license.
  • New graduates are welcome to apply.
  • Experience in skilled nursing facilities is preferred.
  • Completion of a COVID-19 test prior to the start date, with results required within a specified timeframe.
