Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team at Opportunity Healthcare in Nashua, NH. As a Psychiatric Nurse Practitioner, you will be responsible for providing direct patient care, conducting psychiatric evaluations, and managing medication follow-up visits. Responsibilities
  • Conduct psychiatric evaluations and medication management follow-up visits with a 70% productivity expectation.
  • Review records and collateral information in preparation for visits and ensure that 95% of PHQ-9 responses are addressed.
  • Collaborate with nursing staff, medical support staff, and other disciplines to ensure timely and appropriate patient care is delivered.
  • Participate in weekly team meetings, relevant CAFS meetings, and Medical Staff meetings.
  • Complete all documentation within 3 business days, with the first day being the date of service/completion.
  • Promote the quality of patient care through participation in weekly, as needed supervision with the CMO, and maintenance of nursing/medical/psychiatric education.
  • Manage daily requests for medication refills through Essentia Rx.
  • Return all phone calls to patients, family, and/or team members within 48 hours.
Requirements
  • Master's degree in Nursing.
  • APRN Licensed in New Hampshire and DEA Certificate.
  • 1 to 2 years or related work experience.
  • Knowledge of psychiatric diagnoses, medications, and treatments prescribed to clients.
  • Knowledge of relevant nursing principles and procedures.
  • Experience using electronic medical records (EMR) systems.
  • Knowledge of Microsoft Office products (Email, Word, EXCEL).
  • Strong collaborating skills, and an ability to work effectively both independently and as part of a team.
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
  • Excellent communication and interpersonal skills.
  • Ability to multitask and be flexible with regard to workload and assignments.
  • Ability to exercise good judgment and sound decision-making.
