• Deliver high-quality pharmaceutical care to patients in a long-term care setting
• Collaborate with healthcare teams to optimize patient care and medication management
• Ensure accurate record-keeping and compliance with regulatory standards
• Provide medication therapy management and clinical support as needed
• Participate in quality improvement efforts to enhance patient outcomes and satisfaction

• Licensure: Holds or is willing to obtain a Minnesota Pharmacy License
• Certifications: Immunization and CPR certifications are a plus
• Education: PharmD or Medication Therapy Management (MTM) Certification preferred
• Skills: Excellent communication, interpersonal, and collaboration skills
