Job Summary:

The Patient Navigator plays a vital role in guiding patients through the healthcare process, ensuring that the healthcare system meets their needs to the best extent possible. This position involves communicating with patients and their families regarding their procedure, preparation, recovery, and follow-up, as well as communicating with referring physicians. The navigator helps alleviate patients' fears about the procedure by explaining what they can expect and answering all their questions.

Key Responsibilities:
  • Communicate effectively with patients and their families to ensure a smooth healthcare experience.
  • Coordinate with referring physicians to ensure seamless communication and care.
  • Provide clear explanations and answer questions to alleviate patients' fears and concerns.
  • Work closely with the healthcare team to ensure patients receive the best possible care.
Requirements:
  • Bachelor's degree in Nursing.
  • Three (3) years of previous clinical RN nursing or RN patient navigator experience.
  • LICENSES, REGISTRATIONS OR CERTIFICATIONS:
    • BLS.
    • Current RN license or valid permit to practice professional nursing in Texas.
    • Certification in nursing specialty.
  • Preferred Qualifications:
    • A minimum of 1-2 years of Cardiology experience.
