We are seeking a skilled and compassionate Home Health Intake Nurse to join our team at Interim HealthCare. As a Home Health Intake Nurse, you will play a vital role in providing exceptional patient care while enjoying the flexibility and autonomy that comes with this position.

Key Responsibilities
  • Act as the primary point of contact for referral sources, outside agencies, and health professionals to coordinate patient care and services.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Collect and enter patient information into our management information system to ensure accurate and up-to-date records.
  • Work collaboratively with clinical staff to adhere to standards of practice for nursing and applicable laws and regulations.
  • Receive and review orders from referral sources, such as providers, and enter information into the management information system.
Requirements
  • Graduate of an accredited practical nursing program and active LPN license in Colorado.
  • Minimum of one (1) year of LPN experience, preferably in home healthcare.
  • CPR certification.
  • QAPI experience preferred but not required.
  • Able to perform home visits on a PRN basis.
  • Pass a background check and drug test.
  • Knowledge of state and federal home health regulations.
  • Clinical proficiency in nursing, strong communication skills, and compassion.
