The Case Manager will be responsible for managing a caseload of participants in the reentry program, providing support and guidance to help them reintegrate into society. This role requires strong communication and organizational skills, as well as the ability to work with diverse populations.
Key Responsibilities- Manage a caseload of participants, providing individualized support and guidance to help them achieve their goals.
- Conduct risk/need assessments and develop individualized treatment plans for each participant.
- Facilitate evidence-based Cognitive Behavioral Therapy (CBT) programs and other life skills programs to participants groups.
- Document participants' progress and provide regular updates to the Program Manager or Supervising Case Manager.
- Collaborate with other staff members and external agencies to provide comprehensive support services to participants.
- Develop and implement rehabilitative plans to help participants achieve their goals and reintegrate into society.
- Bachelor's degree in a related field, such as social work or psychology.
- Minimum of 2 years of experience working with the criminal justice population.
- Basic knowledge of case management, referral services, and interventions in both group and individual modalities.
- Good presentation, public speaking, and facilitation skills.
- Good organizational skills and attention to detail.
- Effective communication skills with internal and external contacts at all levels.
- Ability to handle multiple priorities simultaneously.
- Problem-solving ability.
- Good interpersonal skills.
- Basic computer skills.
