Job Summary:
Quality Talent Group is seeking a skilled and compassionate Private Duty Nurse LVN to join our team. As a Private Duty Nurse LVN, you will provide high-quality, one-on-one care to patients in their homes, assisting them with transitioning from hospital to home and providing continuous care to remain at home with their loved ones.
Responsibilities:
- Implement and manage patient care plans in accordance with physician orders
- Conduct patient assessments and coordinate care with healthcare teams
- Administer prescribed medication, treatments, and therapies
- Document patient care and progress in a timely and accurate manner
- Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies
- Collaborate with healthcare teams to ensure seamless care transitions and coordination
Requirements:
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
- Six months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
Physical Requirements:
- Ability to speak, write, read, and understand English
- Ability to travel
- Ability to lift 50 pounds
- Ability to sufficiently reposition patients and move equipment without assistance
Work Environment:
- Ability to function in a wide variety of environments, which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids, and infectious diseases
