- Provide palliative care for patients with chronic, complex, and life-threatening conditions
- Assess, educate, and manage symptoms to meet patient needs
- Collaborate with PCPs and specialists to ensure comprehensive care
- Perform Clinical Liaison duties for the Hospice team
- Act as a liaison to explain hospice services and benefits to patients and families
- Participate in community outreach, including marketing and educational sessions
- Attend and participate in staff meetings, educational programs, and community events
- Master's degree in Nursing
- Minimum of five years nursing experience, preferably in oncology, hospice, geriatrics, or intensive care
- Active CNS or Nurse Practitioner license in state of employment
- Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric
- Active NPI number and DEA certification (if applicable)
The demands of this role require regular standing, walking, and manual dexterity. Adaptations can be made to accommodate team members with disabilities. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
