Job Summary

We are seeking a qualified Mental Health Professional to join our Community Support Team as a part of our Assertive Community Treatment (ACT) program. This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore an individual's ability to live successfully in the community.

Key Responsibilities
  • Develop relapse prevention and disease management strategies with individuals
  • Link individuals to clinically appropriate and effective services, including psychological and psychiatric evaluations
  • Provide intensive case management to support individuals in achieving their goals
  • Conduct functional needs assessments to determine the scope and anticipated outcomes of services
  • Assist individuals in finding suitable housing and connecting them to financial and in-kind resources to establish and maintain their households
Requirements
  • Bachelor's degree in Human Services, Social Work, Special Education, or other closely related field
  • One year of professional experience working with the adult mental health population for Assistant Professionals, two years for Qualified Professionals
  • Ability to perform tasks on a computer, including Microsoft 360, Excel, Word, and other basic programs
  • Valid driver's license, auto insurance coverage, and reliable transportation
  • Criminal background check and regulatory clearance
Compensation and Benefits

We offer a competitive salary range of $45,000 - $60,000 per year, based on experience, plus a $3,000 sign-on bonus. Our comprehensive benefits package includes company laptop, 401(k) and Roth retirement plans, 11 paid holidays, direct deposit, paid time off, major medical, dental, and vision insurance, short-term and long-term disability insurance, and life insurance.
