As a RN Case Manager 24hrs Emergency Department, you will play a critical role in ensuring the seamless coordination of patient care, from admission to discharge.
Key Responsibilities:- Facilitate effective communication between patients, families, and healthcare teams to ensure comprehensive care planning.
- Collaborate with multidisciplinary teams to identify and address patient needs, promoting optimal outcomes and resource utilization.
- Develop and implement individualized care plans, incorporating evidence-based practices and patient-centered care principles.
- Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure timely and effective discharge.
- Communicate with payers, administrators, and other stakeholders to ensure seamless coordination of care and optimal resource utilization.
- Current and valid RN license
- 2+ years of experience in Med/Surg, Long Term Acute Care, or similar field
- Strong analytical, data management, and PC skills
- Excellent interpersonal communication and negotiation skills
- Knowledge of third-party payer systems and levels of care
- Current working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement
