Position Overview:

The Home Care Service Coordinator plays a vital role in organizing private duty services for clients, ensuring that staff availability aligns with client needs while maintaining effective communication with both field personnel and clients of various ages. This position guarantees the delivery of safe, responsive, and efficient services.

Key Responsibilities:

  • Oversee essential functions, including but not limited to managing food services and booking conference rooms. Handle incoming mail by collecting, screening, and distributing it, while highlighting critical dates and deadlines.
  • Identify and recruit candidates for open positions through prescreening, interviewing, and conducting pre-employment assessments.
  • Assist in the onboarding process for new hires.
  • Support the Manager in preparing meeting agendas, distributing notices, and recording minutes accurately.
  • Transcribe, photocopy, and disseminate reports, letters, and other required documents.
  • Maintain organized and up-to-date files for easy access while ensuring the confidentiality of sensitive information.
  • Collect referrals from intake and distribute them to the appropriate staff members.
  • Schedule mandatory training sessions for staff.
  • Inform the primary nurse of any issues, service delays, or heavy workloads, and assist in communicating schedule changes to patients and employees.
  • Coordinate the tracking of Home Community Based Care (HCBC) recertifications to ensure timely approvals.
  • Prepare check requisitions and manage supplies and equipment efficiently.
  • Schedule staff according to client requirements.
  • Maintain open lines of communication with field staff.
  • Be available by phone to provide guidance and support during emergencies.
  • Keep accurate records of staff schedules and client service delivery.
  • Collaborate closely with the business office to facilitate payroll processes and resolve any billing issues.
  • Work alongside Nursing Supervisors and Clinical Managers to relay client care plans to field staff.
  • Act as a liaison for non-clinical decision-making and provide input for performance evaluations based on individual and organizational goals.
  • Respond to inquiries promptly and initiate follow-ups on staff or patient concerns.
  • Engage in quality improvement initiatives within the department.
  • Ensure confidentiality regarding employee, legal, client, and company matters.

Additional Responsibilities:

  • Participate in community events and represent MAS when needed.
  • Familiarize yourself with emergency preparedness plans for field staff, particularly regarding natural disasters.
  • Handle equipment and reference materials with care, ensuring routine maintenance is performed.
  • Perform related duties as assigned.

Qualifications:

  • Strong verbal and written communication skills are essential for conveying detailed instructions to patients, families, and colleagues.
  • Ability to receive and interpret detailed information through oral communication.
  • Physical capability to exert up to 20 pounds of force occasionally.
  • Ability to manage periods of high workload and stress effectively.
  • Experience working with frequent interruptions and adapting to unexpected situations.
  • Familiarity with continuous quality improvement strategies is preferred.
  • Proven ability to plan and execute diverse tasks requiring extensive knowledge and sound judgment.
  • Ability to work independently after receiving initial training and guidelines.
  • Strong collaborative skills to foster a positive working environment.
  • Willingness to accept constructive feedback and take appropriate corrective actions.
  • Ability to maintain effective communication and relationships with colleagues, clients, and the public.
  • Availability to work weekdays, with occasional shifts on weekends and holidays as needed.

Education and Experience:

Education: High school diploma or equivalent with a focus on secretarial or office management training.

Experience: Minimum of one year in a business or medical office setting.

Preferred Certification: LNA/CNA/PCA/PSS/PCSP.

Technical Skills: Proficiency in EMR and Microsoft Office 365.
