Job Description

We are seeking a compassionate and skilled Medical Social Worker to join our team at Paloma Hospice and Palliative Care. As a key member of our multidisciplinary team, you will play a vital role in providing emotional support and guidance to our patients and their families.

Responsibilities:

  • Develop and implement personalized support plans to address the unique needs and challenges of our patients.
  • Conduct thorough assessments to identify areas of strength and weakness, and develop targeted interventions to promote optimal well-being.
  • Collaborate with our healthcare team to ensure seamless care coordination and effective communication.
  • Provide education and counseling to patients and families on coping strategies, stress management, and end-of-life care.
  • Maintain accurate and detailed records of patient interactions and progress.
  • Participate in case conferences and provide expert testimony as needed.
  • Develop and maintain professional relationships with community resources and services to ensure access to comprehensive care.

Requirements:

  • Master's degree in Social Work (MSW) or related field.
  • Current licensure as a Licensed Medical Social Worker (LMSW) in the State of Texas.
  • Proven experience in hospice and palliative care, with a strong understanding of social theories and practices.
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with patients and families.
  • Ability to work effectively in a fast-paced, dynamic environment, with a strong commitment to teamwork and collaboration.

Work Environment:

  • Full-time position, Monday through Friday, 8:30am - 5:00pm.
