Job Summary

Key Responsibilities:
  • Facilitate coordination and development of comprehensive and individual plans of care in collaboration with patients, families, and multidisciplinary teams.
  • Initiate referrals to social services and communicate with necessary disciplines and agencies.
  • Act as a liaison for interdisciplinary communication and coordination of discharge plans.
  • Utilize approved departmental communication tools for documentation.
  • Provide individualized care planning consistent with efficient and thorough assessments of patients.
  • Provide follow-up with patients/families during hospitalization for continuity of care.
  • Act as a liaison with agencies and patients for potential adoption cases.
  • Provide consultation to case managers.
  • Ensure and maintain consensus related to discharge plans from patients, families, physicians, and payors.
  • Collaborate with nursing staff, case managers, ancillary staff, physicians, facilities, and community agencies to identify patient/family educational needs.
  • Communicate with patients/families to ensure understanding of third-party payor guidelines.
  • Document utilization activities and findings based on departmental standards.
  • Facilitate ongoing updates to databases to assist with efficient delivery of medical care.
Requirements:
  • Bachelor's Degree in Nursing/Social Work with a minimum of 3 years of nursing/Social Work experience in medical/surgical, critical care, and/or hospice, home health, or public health required.
  • Associate Degree in Nursing with a minimum of 4 years of experience in medical/surgical, critical care, and/or hospice, home health, or public health required.
  • Master's degree in nursing/master's degree in social work or related field preferred.
  • Case Management certification preferred.
