About the Role:
The Clinical Pharmacy Specialist will be an integral part of our Quality Pharmacy Operations team, which collaborates closely with WellMed Medical Management. This role is pivotal in delivering patient-focused, clinically-informed medication consultations aimed at enhancing health outcomes.
Key Responsibilities:
- Conduct comprehensive telephonic medication consultations that meet or exceed established productivity and quality benchmarks.
- Perform thorough annual medication reviews, focusing on both prescription and over-the-counter medications, while emphasizing patient assessment and counseling.
- Evaluate patients' claims, medical histories, and health conditions to provide tailored pharmacotherapy assessments.
- Identify barriers to medication adherence and develop strategic plans to enhance patient compliance.
- Offer evidence-based therapeutic recommendations to healthcare providers.
- Document all clinical consultations accurately in our internal database.
- Engage in continuous professional development through participation in staff meetings and interdepartmental collaborations.
- Stay updated on chronic disease management and pharmacy practice advancements to serve as a reliable resource for medication utilization.
Qualifications:
- Doctor of Pharmacy degree or equivalent experience.
- Active pharmacy license in your primary state of residence, with the ability to obtain licensure in additional states.
- A minimum of 3 years of experience in Clinical Pharmacy Practice.
- Experience in a call center environment is preferred.
- Familiarity with Medicare STARs and HEDIS measures.
- Proficiency in Microsoft Office Suite.
- Strong communication and interpersonal skills, with the ability to manage multiple projects effectively.
Preferred Qualifications:
- MBA or MPH degree.
- Postgraduate training such as a Pharmacy Residency or Fellowship.
- Board certification by the Board of Pharmacy Specialties.
- Bilingual candidates are encouraged to apply.
