Key Responsibilities
  • Conduct thorough assessments of client needs and develop comprehensive service plans to address their requirements.
  • Collaborate with clients, family members, and community resources to ensure seamless delivery of services.
  • Monitor and evaluate the effectiveness of service plans, making adjustments as needed to optimize outcomes.
  • Maintain accurate and detailed records of client interactions, service plans, and outcomes.
  • Communicate effectively with clients, colleagues, and external stakeholders to ensure timely and accurate information exchange.
  • Participate in multidisciplinary teams to implement behavioral health services and treatment plans.
Requirements
  • 6 months of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
  • 6 months of experience in making recommendations as part of a client's service plan, such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
  • 6 months of experience in interpreting laws, rules, regulations, standards, policies, and procedures.
  • 6 months of experience in using an automated information system to enter, update, modify, delete, retrieve/inquire, and report on data.
  • 6 months of experience in narrative report writing.
