Pharmacist Part Time Benefits Eligible Mineola

We are seeking a highly skilled Pharmacist to join our team in a part-time, benefits-eligible position at our Mineola location. As a Pharmacist, you will be responsible for all aspects of medication therapy, including review of inpatient medication orders and outpatient prescriptions, dispensing of medication, and counseling of patients.
Responsibilities:- Enter medication information orders appropriately (SIGs, Start/Stop Dates, etc) from written orders D/C's discontinued medications. Validate CPOE orders, and make necessary corrections to order. Screen for possible adverse drug events. Document interventions, notes, and allergies properly.
- Perform other duties as assigned
- Convey pertinent issues to the appropriate personnel to maintain an effective continuum of patient care and make appropriate documentation for efficient follow-up. Contact prescribers directly to resolve medication and therapy issues. Notify Patient care personnel of outstanding issues affecting their patients. Inform outstanding issues to Pharmacy personnel working current and out coming shift. Interventions, ADR's note and other documentation is entered into the computer appropriately.
- Handle controlled substances in accordance with federal, state, local regulations, and hospital policies and procedures. Process wasted, returned and expired drugs. Resolve discrepancies and notify management of those discrepancies.
- Maintain accurate patient profiles, prescription records, including patient's name, DOB, address, and date filled, initials or signature, etc. Screen prescriptions for appropriateness of drug and therapy, and possible adverse event potential. Dispense drug with the appropriate auxiliary labeling. Offer counseling to patient.
- Comply with policies and procedures related to working in the sterile preparation area; proper hand-washing procedure; special handling requirements and procedures for preparation of sterile bio-hazardous orders. Maintain clean and aseptic workspace; Handle and dispose of sharps, bio-hazardous materials in a safe manner. Review chemo orders for appropriate drug, dose, protocol, and regimen. Receive co-signatures on orders; Maintain proper records for reference on patients for future treatments, for Human Resources regarding personnel handling bio-hazardous substances.
- Participate in the training and supervision of staff, and continues to be a mentor and knowledge base for the purpose of enhancing continual staff development. Review filled orders for accuracy. Correct any errors and educate personnel to the nature of the error and preventative measures.
- Ensure patient-safe, time effective and appropriate medication therapy. Triage and review orders for appropriateness of therapy. Select correct drug/dose/dosage form to be labeled. Prepare accurate container label, appropriate container and affix the proper auxiliary information.
- Bachelor's Degree in Pharmacy
- Current New York State Pharmacy License
- Excellent communication (both oral and written), organizational, detail oriented, problem solving, and record keeping skills
- Basic computer skills
- Proficient in the use of the Pyxis system
- Knowledge of the special handling requirements and additional policies and procedures for preparation of sterile bio-hazardous orders
- Post Doctoral PharmD
- 1-2 years Licensed Pharmacist in healthcare or retail environment
