- Follow-up Support: Provide ongoing support and connection with individuals in the community, ensuring follow-up with agreed-upon referral sources and continued education on mental health resources.
- Treatment Goal Setting: Participate in establishing treatment goals, parameters, and services that contribute to positive consumer outcomes in the community.
- Documentation and Reporting: Accurately document needs assessments, goals, and progress towards stability, adhering to agency standards.
- Flexibility and Adaptability: Be prepared to work a flexible schedule to ensure continuity of care and optimal support services.
- 1-2 years of prior experience in peer support or a related field
- Current certification in peer support preferred
