We are seeking a highly skilled and compassionate Hospice Nurse Practitioner to join our team at SouthernCare. This per diem opportunity is perfect for individuals who want to make a meaningful difference in the lives of patients and families during their end-of-life journey.

As a Hospice Nurse Practitioner with SouthernCare, you will be responsible for performing Face-to-Face (F2F) encounters and gathering clinical information to support hospice diagnosis and prognosis. You will communicate your findings to the hospice physician responsible for certifying or recertifying patient eligibility.

The scope of practice for this role is limited to tasks necessary to complete F2F encounter visits. Key responsibilities include:

  • Performing physical assessments for F2F encounters within regulatory time frames established by CMS
  • Providing clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists to the certifying physician
  • Making recommendations to the Interdisciplinary Group (IDG) based on assessment findings
  • Notifying the hospice agency when unable to complete a scheduled F2F visit

To be successful in this role, you will need:

  • A Master's degree in Nursing
  • Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body
  • A current state license as a registered nurse and advanced practice nurse
  • National certification as a Nurse Practitioner (ANCC or AANP)
  • A collaborative agreement in place
