Aveanna Healthcare Private Duty Nurse LVN

As a Private Duty Nurse LVN, you will play a vital role in delivering high-quality care to our patients, working closely with our multidisciplinary team to ensure seamless care coordination.
About the RoleWe are seeking a skilled and compassionate LVN to join our team in providing private duty nursing services to our patients. As a Private Duty Nurse LVN, you will be responsible for implementing the physician's prescribed plan of care, conducting patient assessments, and coordinating care with our team of healthcare professionals. You will also be responsible for administering prescribed medication, treatments, and therapies, as well as documenting patient care in our electronic system of record.
Responsibilities- Implement the physician's prescribed plan of care, ensuring that all necessary treatments and therapies are administered in a timely and effective manner.
- Conduct patient assessments, identifying any changes in the patient's condition and communicating these changes to the healthcare team.
- Coordinate care with our multidisciplinary team, including physicians, therapists, and other healthcare professionals.
- Administer prescribed medication, treatments, and therapies, as well as document patient care in our electronic system of record.
- Provide health promotion, teaching, and training to family members, ensuring that they are equipped to provide ongoing care and support to their loved ones.
- Active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice.
- Current CPR certification from American Red Cross, American Heart Association, or BLS.
- TB skin test (current within last 12 months).
- Six months prior hands-on nursing experience preferred but not required.
- Must have reliable transportation.
