Job Summary: We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality medical care for residents in a Long-Term Care Facility/Nursing Home setting. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering patient-centered care.

About the Role: As an LPN Medical Care Specialist, you will be responsible for supervising nursing personnel, administering medications, taking vital signs, and performing basic diagnostic tests. You will also observe, evaluate, and report changes in patient symptoms, condition, and reactions to medications or treatments. Your attention to detail, ability to multitask, and commitment to providing exceptional care will make you an invaluable member of our team.

Responsibilities:
  • Provide direct patient care within the scope of the Practical Nurse Act of Pennsylvania.
  • Communicate effectively with healthcare teams, patients, and families to ensure seamless care coordination.
  • Administer medications, take vital signs, and perform basic diagnostic tests as ordered by healthcare providers.
  • Maintain accurate records, document patient information, and update treatment plans as necessary.
  • Collaborate with healthcare staff to identify and address patient needs, ensuring a culture of safety and quality care.


Qualifications: To be successful in this role, you will need:
  • A valid Licensed Practical Nurse (LPN) license.
  • Current CPR/BLS certification.
  • Valid TB testing documentation.
  • A current physical examination.
  • One year of nursing experience is preferred but not required.
