Job Summary

Monument Health is seeking a skilled Wound Ostomy Nurse to join our team. As a key member of our healthcare team, you will be responsible for coordinating patient services, case management, and providing direct skilled care to patients with wound and ostomy needs.

Key Responsibilities
  • Assess patients' needs and develop individualized care plans to promote optimal outcomes.
  • Provide education and training to patients and caregivers on wound and ostomy care.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination.
  • Initiate and document educational needs, providing resources to patients, families, or caregivers to meet their learning needs.
  • Pre-operatively select potential stoma sites based on patient anatomy, activity, and lifestyle, and initiate pre-op teaching as requested.
  • Prioritize and organize delivery of care in a consistent manner, initiating interdisciplinary collaboration to promote optimal patient care and outcomes.
Requirements
  • Certification - Registered Nurse (RN) - South Dakota Board of Nursing
  • Preferred: Experience - 1+ years of Registered Nurse Experience; 1+ years of Wound Care Experience
  • Education - Bachelors degree in Nursing
  • Certification - Wound Care Certification/Specialist - Accredited University or accredited training professionals
