Key Responsibilities:

  • Identify and assess the psychosocial needs of members, providing guidance and support to help them navigate the healthcare system.
  • Develop and implement individualized care plans that address the member's physical, emotional, and social needs.
  • Collaborate with healthcare professionals to ensure seamless coordination of care and services.
  • Provide education and support to members and their families to promote healthy behaviors and self-management skills.
  • Conduct regular assessments and evaluations to ensure the member's needs are being met and make adjustments to the care plan as needed.

Requirements:

  • Master's degree in Social Work (MSW) required.
  • Minimum three (3) years of clinical experience plus two (2) years in case management required.
  • Minimum one (1) year of experience working with the defined population required.
  • Licensed Clinical Social Worker - Certified (Maryland) within 6 months of hire.
  • Independent Clinical Social Worker License (District of Columbia) within 6 months of hire.
  • Licensed Clinical Social Worker (Virginia) within 6 months of hire.
  • Case Manager Certificate within 36 months of hire.

Preferred Qualifications:

  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.
  • Clinical social worker experience preferred.
