Key Responsibilities:
- Patient Assessment: Conduct thorough assessments of patients' health conditions, including physical examinations, medical histories, and reviewing diagnostic test results.
- Diagnosis and Treatment: Diagnose and treat a wide range of acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications or therapies as needed.
- Preventive Care: Provide preventive care services, including vaccinations, health screenings, and lifestyle counseling to promote overall health and wellness.
- Health Education: Educate patients and their families on health conditions, treatment options, medications, and self-care techniques to support optimal health outcomes.
- Care Coordination: Collaborate with physicians, nurses, specialists, and other healthcare professionals to ensure coordinated and comprehensive patient care.
- Telehealth: Deliver virtual healthcare consultations, referrals, and follow-up care through our telehealth platform, ensuring seamless patient access to services.
- Record Keeping: Maintain accurate and timely patient records in compliance with healthcare regulations and standards.
- Prescription Management: Monitor and manage patients' medication regimens, including adjusting dosages and renewing prescriptions as necessary.
Requirements:
Education:
- Hold a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
- Licensure: NP: Possess a valid and unrestricted Nurse Practitioner license in the State of New York.
Language Skills:
- Fluency in English in addition to Mandarin or Cantonese is required to effectively communicate with our diverse patient population.
Certification:
- Must be certified by a recognized certifying body, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
- Clinical Skills: Demonstrated proficiency in conducting patient assessments, diagnosing medical conditions, and developing and managing treatment plans.
- Communication Skills: Strong interpersonal and communication skills to effectively interact with patients, families, and other healthcare providers.
- Flexibility: Ability to work within multiple telehealth and remote systems to deliver comprehensive and high-quality care to patients.
- Continuous Learning: Stay up-to-date with medical advancements, healthcare guidelines, and best practices through continuing education and professional development.
- Cultural Competency: Provide culturally sensitive care and show respect for diverse patient populations.
- Technological Proficiency: Familiarity with electronic health record systems (EHR) and telehealth technology is required.
