Key Responsibilities
  • Conduct in-home health evaluations, focusing on patients' overall health and well-being, and identifying potential barriers to care.
  • Work with patients to develop personalized care plans, taking into account their unique needs and circumstances.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Earn up to $2,500 to $4,000 per week, with opportunities for additional compensation through our pay-per-test model.
