We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Ascension. As a Registered Nurse, you will provide direct nursing care to patients in their homes, working collaboratively with our interdisciplinary team to ensure high-quality patient outcomes.
Key Responsibilities- Provide Direct Patient Care: Implement and monitor patient care plans, monitor and record patient condition, and communicate with healthcare team members as needed.
- Coordinate Patient Care: Serve as a primary coordinator of all disciplines for well-coordinated patient care, assessing and coordinating patient's discharge planning needs with the healthcare team.
- Manage Patient Records: Note and carry out physician and nursing orders, maintaining accurate and up-to-date patient records.
- Licensure and Certification: Current licensure as a Registered Nurse in the state of Illinois, BLS Provider certification, and a valid Illinois driver's license.
- Education: Diploma from an accredited school of nursing or required professional licensure at time of hire.
- Home Care Experience: Previous experience in home care or acute/post-acute settings, with a background in patient assessment and care planning.
- Availability: Monday - Friday, 8:00am - 5:00pm, with on-call, weekend, and holiday rotation.
