Engagement Navigator

At AcuteCare Health System LLC, we are committed to providing exceptional care and services to our participants. As an Engagement Navigator, you will play a vital role in building strong relationships with prospects, their families, caregivers, and community stakeholders to ensure successful intake and enrollment into our PACE program.
Key Responsibilities:- Effectively explain the PACE program, eligibility criteria, and enrollment processes to potential participants and their families.
- Schedule and facilitate assessments to ensure a smooth enrollment process.
- Communicate effectively with team members, adhering to person-centered processes and tracking pertinent data and statistics.
- Develop and maintain strong relationships with community stakeholders, including government agencies, community members, and referral sources.
- A baccalaureate degree in healthcare administration, business, communications, marketing, or a related human services field is preferred.
- High School diploma or equivalent is required.
- Experience in community outreach, intake, enrollment, or marketing is preferred.
- Reliable transportation, a valid driver's license, and minimum state required liability auto insurance are required.
- Medical clearance for communicable diseases and up-to-date immunizations are required before engaging in direct participant contact.
- A comprehensive criminal background check is required, including federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
- A people-first approach to all decision making, planning, and program practices.
- Effective communication expertise with staff, colleagues, and clients in various settings.
- Demonstrated commitment to promoting quality services to a frail client population with a focus on continuous improvement.
- Ability to be creative and innovative in developing systems and making decisions.
- Understanding of the principles of diversity and inclusion and ensuring their incorporation into the culture, policies, and practices of the outreach and engagement department.
- Ability to maintain a consistent and clear adherence to established, person-centered processes to promote continued census growth and services to the community.
- Skills to effectively represent the program to participants, families, outside agencies, community groups, community professionals, and the general public.
- Effective relationship-building skills with potential enrollees, government agencies, community members, referral sources, and co-workers.
- Knowledge of federal, state, and local regulations and requirements related to PACE outreach and engagement practices.
- Ability to maintain accurate records and prepare clear and concise reports, correspondence, and other written materials.
- Excellent verbal and written communication skills.
- Public speaking skills for large and small groups.
- Familiarity with management information systems and electronic health records.
