Job Summary

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and providing personalized care plans to improve health outcomes.

Key Responsibilities:

  • Conduct home visits to assess patients' health and identify potential risks
  • Develop and implement personalized care plans to address identified health risks
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and self-management

Requirements:

  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a relevant specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong communication and interpersonal skills
