About the RoleAs the RN Case Manager, you will be responsible for the delivery of the Plan of Care for each hospice patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.
You will make the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral.
You will identify the patient's/family's physical, psychosocial, and environmental needs and re-assess as needed, no less than every fifteen (15) days.
You will initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
You will document problems, appropriate goals, interventions, and patient/family response to hospice care.
You will collaborate with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.
RequirementsWe are looking for someone with one year of clinical experience.
Current RN licensure in the state of practice and one year of clinical experience are required.
Current CPR Certification is also required.
Additionally, you should have a current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
