Responsibilities
- Scheduling: Prepare weekly schedules and conduct home visits as assigned. Prioritize nursing visits based on client needs and the organization's emergency preparedness protocols.
- Assessment and Documentation: For each new patient, create a personalized and comprehensive care plan in collaboration with the patient and their family. Conduct thorough physical and psychosocial assessments to inform the care plan.
- Ongoing Assessment: During each visit, demonstrate the necessary skills and judgment to execute the care plan and nursing interventions, as evidenced by chart reviews and direct observation. Continuously update the care plan as goals are achieved and new challenges arise.
- Discharge Planning: Ensure that discharge documentation reflects improvements in patient outcomes or identifies any variances. Prepare patients adequately for discharge, ensuring all care team members agree on the discharge plan.
- Medication Management: Exhibit knowledge of departmental policies regarding medication administration and documentation. Reconcile medications during each visit and update the medication profile as necessary. Educate patients and their families about medication management, including potential side effects.
- Case Management: Take responsibility for coordinating the care plan for each patient on your caseload, involving the patient and their family in the process. Communicate effectively with all team members to ensure high-quality care.
- Safety Practices: Promote a culture of safety through personal accountability and commitment to maintaining a safe environment. Adhere to current hand hygiene and infection control protocols.
- Professional Growth: Engage in opportunities to maintain and enhance your professional nursing image.
- Leadership Opportunities: Participate in formal or informal leadership roles.
- Advanced Clinical Skills: Take part in opportunities to demonstrate and expand your advanced clinical skills and knowledge. Commit to your professional development by integrating evidence-based practices and research into your clinical work.
Graduate from an accredited School of Nursing. A Bachelor of Science in Nursing from a NLN-accredited institution is preferred.
Experience:
One to two years of medical/surgical nursing experience is preferred.
Must be proficient in reading, writing, and speaking English. Basic nursing competencies are required.
